We welcome new patients. Please take the time to review the following information which can make the transition of your care to our practice easier.

Transfer of care may require the following steps:

Inform your insurance company of your plan to change your primary care physician and ask your insurance case manager to assign Dr. Kousa as your new PCP.

Download, print and fill out the patient registration form, which you can download here.
Download, print, fillout and mail to the physician you are leaving, a patient transfer form, which you can download here.

On your first visit to our practice, bring with you the following:

Photo ID (driver license).
Insurance Card.
Patient registration forms, completed.
Your medications, or list of them.
Log books for your blood pressure, blood sugar, diet.
Any due copays and deductibles.
