We make every effort to schedule an appointment at a time that is convenient for you.
Making appointments:
You do not need a physician referral to be seen by our physicians. To make an appointment, it is important that you call in advance to leave yourself enough time to work the appointment in your schedule
and avoid conflicts. This can also help you get the care you need ontime.
Preparing for your appointment:
It is important to bring with you all your medications, any test reports or X-rays that were done and any log books for your blood pressure, blood sugar or weight. It is also important to review your insurance policy and learn the stipulations of your health plan, your benefits and responsibilities. This will help you prepare for any testing your doctor may order. It is also important that you know you coverage limits and be prepared to pay your deductible or copay at the time of service. If you have an insurance plan which restrict your access to one primary care physician, then make sure you call your case manager to confirm that Dr. Kousa is assigned by your insurance as your primary care physician.
Often it is helpful for family members or caregivers to come in with patients who are very debilitated or have significant memory impairment.
